Advance Directives

The policy at High Plains Surgery Center is that in the event a patient presents an Advance Directive or Living Will to one of our staff members, the staff member will inform the patient that any DNR (Do Not Resuscitate) orders will not be honored during the patient’s visit. This conversation will be documented in the patient’s medical record. The staff member will also notify the Medical Director, Administrator, or designee.

A copy of the Advance Directive or Living Will will be placed in the patient’s chart and included with the center’s medical record in the event the patient is transferred to another healthcare facility as part of his or her visit to High Plains Surgery Center.
